Scott Salus, Occupational Therapist, Owner

Scott earned his Master of Science in Occupational Therapy from Springfield College in 2016. He has worked with older adults for his entire career, first in inpatient rehab, and he has worked in home care since 2017. 

Scott knew before he began his schooling that he wanted to work with the geriatric population, and he continues to be fascinated by the ways OT can dramatically improve the lives of his senior clients. His approach is holistic, always keeping in mind each client’s specific difficulties, joys, home environment, history, and preferred activities.

Scott’s specialties include dementia, Parkinson’s disease, chronic pain, mental health conditions, and injuries of the hand, arm, shoulder and neck. He is certified in LSVT BIG, an exercise program specifically for people with Parkinson’s. He can recommend wheelchair modifications, home equipment, exercise programs, and mindfulness techniques.

He became an Occupational Therapist because he witnessed the wonderful ways an OT was able to improve the life of his grandfather when he was diagnosed with dementia. He adores working with the senior population and strives to provide the kind of care his grandfather received.

Fun Facts

Scott moved from Brooklyn, NY to Colorado in 2020 with his partner Eri and their sweet dogs Remy and Banana. He is a lifelong musician - he sings, plays guitar, and drums. He has recently become certified in integrative sound and music healing at the Open Center in New York City. Scott has been a Reiki Master Practitioner since 2015. Scott and his partner welcomed their daughter, Georgia Heart in 2023.

Daniel Basta, Physical Therapist, Owner

Daniel obtained his Master in Physical Therapy from the College of Staten Island, New York in 2008. He has been a physical therapist for the past 14 years; he started his career working for 4 years in a hospital setting, and he then transitioned to private practice for 1 year. Since 2013, Daniel has been a home care physical therapist.

Daniel fell in love with home care because of the one-on-one time that he gets to spend with his clients. He enjoys getting to know each person on a deeper level and how they interact in their home environment. Daniel enjoys learning his clients’ life stories, as well as those of their families. This helps him make physical therapy a team approach.

Daniel loves his profession and the rewarding feeling of improving his clients’ lives. His specialties include orthopedic, neurological, cardiopulmonary, and general medical conditions, mainly in the senior population. He is an advocate for preventative care and assesses gait, balance, and movement of each of his clients to help them stay upright and prevent injury or falls.

Daniel’s inspiration is his grandmother, who greatly benefited from physical therapy and fully recovered from a fractured hip.

Fun Facts

Daniel is a Brooklyn, NY native. Before moving to Colorado, Daniel spent 5 years in Orlando, where he met his wife. Both love the outdoors, nature, hiking, snowboarding, cooking and spending time with family and friends. Colorado is the perfect home. They had their first child Violet at the end of 2020, and their second, Lily in 2023.

Well traveled, he has been to almost every state and has visited many countries internationally. He has been coming to his parents’ summer home in Boulder every year since he was a child. 

Amanda Bauer, Occupational Therapist

Amanda graduated from West Virginia University with a bachelor’s degree in health and human performance in 2006 and subsequently completed her master’s degree in occupational therapy in 2007.  Since then, while working in multiple areas of practice from a level 3 trauma center’s ICU to in-home rehabilitation (and everything in between), she has fallen head-over-heels for the profession and the hope it provides to its clients. 

Amanda grew up attending therapy appointments with her younger brother and knew from an early age that OT was her future.  Initially, she planned to work in pediatrics but took a quick turn towards the senior population during her first shadowing experience in the setting.  She has worked with adults of all ages since, with a special place in her heart for the golden-aged.  It Is her belief that every human being has a valuable history and deserves to live their life to its fullest potential, even when their ability to do so begins to change.

There is nothing about occupational therapy that doesn’t bring her joy, but Amanda’s specialty areas are aging in place and mental health.  She has been working in in-home care for the past 3 years and has enjoyed seeing clients regain independence and contentment in their lives. 

Fun Facts

In general, Amanda is a devourer of books, daring cook and baker, up-and-coming crazy cat lady, and doting mother to 2 brilliant boys. Her husband and she moved their family to Colorado in 2015 for a new adventure and they’re enjoying the quintessential Coloradan life.

Kim Larriviere, Occupational Therapist

Kim received her Master of Occupational Therapy degree from Louisiana State University in 2016. She worked in an outpatient therapy setting for 5 years before moving into home care. She was first drawn to occupational therapy as a teenager when she saw its role in helping children achieve greater functional ability and independence in everyday life.

So far, Kim adores working in seniors’ homes because it provides a unique opportunity to work in her client’s natural environment. This allows her to address the needs of clients in relation to their everyday life at home so they can live safely and productively. She also loves working in the home setting because it allows her to work with clients who may face challenges getting to a clinic.

Kim specializes in neuro-rehabilitation. She is certified in LSVT BIG, an exercise program for people diagnosed with Parkinson’s disease, and in Neuro-IFRAH, a treatment approach for people with a stroke or brain injury.

Fun Facts

Before moving to Colorado, Kim lived in Louisiana and Texas. Her favorite hobbies include reading, cooking, trying new foods, skiing, spending time with family, and camping with her husband.

Gabi Ketzenbarger, Physical Therapist Assistant

Gabi was born and raised in Keystone, Colorado. She attended CU Boulder for her undergrad years, and from there, pursued further education at the PIMA Medical Institute for more specific physical therapy assistant training. She started her career in an outpatient orthopedic facility but quickly transitioned into home health therapy after about a year of working in the field. While she is newer to the physical therapy world, she is incredibly passionate about the field and about helping others as best she possibly can!

She is also very passionate about the outdoors and the mountains. She spends the majority of her free time rock climbing, mountain biking, backpacking or skiing, usually alongside her fiance and 9-year old pup (who can still adventure with the best of them despite her age!).

Sammi Curry, Physical Therapist

Sammi believes in a personalized approach to physical therapy care, taking the time to understand her patients’ concerns and goals. Her extensive knowledge and expertise in the areas of orthopedics, neurology, and gerentology allow her to offer a wide range of treatments to her clients, providing for a holistic plan of care. Sammi is dedicated to fostering a nurturing and welcoming environment for her patients, ensuring they feel seen, heard, and valued in their healthcare experience.

After completing her Masters Degree in Physical Therapy in 1997 at the University of Colorado‘s Health Science Center, she began practicing in orthopedics, then moved into home health. She is passionate about educating individuals on the importance of functional strengthening and healthy practices, and about helping them achieve optimal wellness and functional mobility in their daily lives.

Beyond her professional life, she finds joy in the outdoors. Passionate about taking every opportunity to seek out an adventure, she volunteers in national adaptive sports events and extensive bike touring experiences. Closer to home, she enjoys gardening with a focus on raising heirloom tomato varieties and native plant landscaping. She loves hiking in the Foothills, camping, and kayaking. During the colder months, she can be found exploring new recipes in the kitchen and hunting for her next warm weather vacation.

Fun Fact

As the sixth generation of her family to be born and raised in Hawai’i, Sammi takes to heart the spirit of ‘aloha’ that has shaped her outlook on life and her approach to her clients.  Aloha is more than a word of greeting or farewell, or a farewell, or a salutation. Aloha is the essence of relationships in which each person is important to every other person for collective existence.

Kelly Fox, Physical Therapist Assistant

Kelly has lived in Colorado for almost 20 years and is originally from Minnesota.  She came to Colorado during college as a transfer student and graduated from the University of Colorado with a Bachelor’s Degree in Human Resources Management.

During college, Kelly worked as a nanny in the Boulder area and that's when her desire to care for people emerged. After working in the HR corporate setting for a few years, she decided to follow that desire and became a Certified Nurse Assistant. Eventually, her mind was made up that healthcare was her calling, so she quit her corporate job and went back to school to become a Physical Therapist Assistant. She has skilled nursing and rehab experience, but much of her work as a PTA has been with the geriatric population in the home health setting.

When she's not working, Kelly enjoys crafting and playing games with her young daughter. She is an avid hiker and has explored many beautiful trails Colorado has to offer, including conquering one 14er.  She looks forward to continuing to learn and grow as a PTA with Kind Rehabilitation.

Fretche Blair, Physical Therapist

Fretche is a proud Filipino immigrant who earned her Bachelor in Physical Therapy from Southwestern University, Cebu, Philippines in 1999. She started her journey as a Physical Therapist in the US in 2003. Her passion for learning and pursuit of excellence inspired her to obtain her Doctorate Degree in Physical Therapy and completed it from University of Montana in July 2023. 

She is a Certified Exercise Expert for Aging Adults and a Certified Stroke Rehabilitation Specialist. She serves as a Board Trustee of FUTURE Foundation, a nonprofit organization which aims to advocate for the PT Profession globally. She is an active American Physical Therapy Association (APTA) member), APTA and APTA Colorado Geriatrics member and APTA Acute Care member. 

Her clinical experience includes skilled nursing facilities, outpatient, home health, inpatient rehabilitation and acute hospital care settings. Fretche is a passionate and a strong champion for the older adults and believes in empowering them to achieve a great quality of life.

Fretche’s clinical areas of interest are Orthopedic, Stroke, Traumatic brain injuries, Parkinson’s, Chronic pain, Cardiopulmonary, General Medicine  and anything related to the Geriatric population.

Despite her busy professional and family life, Fretche is an avid hiker. Her fervor for hiking is paralleled by her love of traveling. She loves food, books and dogs.

Donna Rainford, Occupational Therapist

Donna earned her Bachelor of Science degree in Occupational Therapy from Colorado State University in Ft Collins in 2001. She has been working in outpatient and inpatient rehabilitation for 20 years focusing on neurological injuries. She has recently transitioned to home care, collaborating with geriatric clients and their families. Donna has experience with recommending equipment for home, exercise programs and wheelchair needs.

Donna especially enjoys working with seniors to help improve their quality of life and return to what they enjoy. She loves helping people adapt to life changes and finding effective ways of meeting individual needs. The concept of aging in place is something she is passionate about and finds working in a home environment the best way to accomplish client goals.

Donna grew up in the Boulder area and loves to travel and spend time outdoors hiking, biking, and taking walks with family and friends.

Lynn Herklotz, Occupational Therapist

Lynn is a Colorado Girl. She was born, raised, and received her Occupational Therapy, Massage, and Lymphedema Therapy trainings in Colorado. She has been practicing in the field of physical rehabilitation and wellness since 1989, and has shared her knowledge as an instructor and mentor.

Lynn has a deep appreciation of the outdoors and spends her free time enjoying the fresh air and sunshine. Her desire to improve her clients’ quality of life is at the core of her Occupational Therapy practice.

Melissa Blackwell, Physical Therapist Assistant

Melissa Blackwell is a licensed Physical Therapist Assistant and received her Associate's Degree in Science from North Shore Community College’s Physical Therapist Assistant program in 2019, after working in Hospitality and Client Services for the past two decades. She also holds a Bachelor of Science Degree from Sierra Nevada College.

Since graduating she has been helping patients in various clinical settings such as vestibular, orthopedic, pediatrics and home care. Melissa enjoys working with patients of all ages and levels, and loves utilizing her skills to improve patient function. Her passion is working hands-on with people, to help them to recover their independence and improve their quality of life.

Melissa loves the outdoors and if she is not spending time with her husband and two daughters she is hiking, skiing or at a yoga class.
